Can I see Dr. Holzwarth even if I have a referral for somewhere else or am actively seeing another therapist?Yes! most physician offices have branded referral pads for one reason or another, but that referral is good for any PT clinic or encounter. Dr. Holzwarth frequently sees patients from other providers who want a more thorough and detailed assessment. He is happy to write a written report for you to take back to your provider. As if often good practice with getting multiple opinions when a surgery is recommended, it is very helpful to have a second opinion on your therapy plan of care to ensure you are working on and addressing the correct items.

Do I need a referral to come see Dr. Holzwarth?No! As Doctors of Physical Therapy, we are able to see patients directly without referrals. In the state of Georgia, we are able to work with patients for 8 visits of 21 days prior to having a signed plan of care from one of your providers. If the need should arise, we will navigate that process for you.

Do you accept Insurance at Equalizer HealthWe are an out-of-network provider and collect payment at time of service. You will have access to your "superbill" (or medical care receipt) through your patient portal, that has all the appropriate documentation for you to submit to your insurance carrier for reimbursement. This allows us to treat individual patients at a fraction of the cost because we do not have the overhead of coding and billing personnel and services. This also allows us to be completely transparent in cost of care so that there are NO surprise medical bills that you need to worry about. In-network billing rates for Physical Therapy for a single session around metro Atlanta are between $450-650 and if you have yet to meet your deductible, those full costs get passed on to you while still having much lower quality of care. We in the PT profession refer to these clinics as "patient mills" because they are all about volume. Get as many patients in at a time with as few providers as possible and using as few resources as possible.

What would the initial evaluation look like with Dr. HolzwarthPlease wear something comfortable and loose fitting. Following introductions and clinic tour, we will begin with a thorough history of your reason for the visit including but not limited to medical, exercise, nutritional and lifestyle history.
