Do I need a referral?
No, you do not have to have a referral. Dr. Holzwarth can work with patients directly in the state of Georgia even if they’re seeing another provider or have a referral somewhere else; getting multiple assessments and opinions can be very helpful, especially when the option of surgery is involved. Our physical therapists are happy to write detailed reports to take back to your own provider if you’d prefer.
Do you accept insurance?
No, we are considered an out-of-network provider, so payment is required at the time of service. This model allows for transparent pricing with no surprises and a higher quality of care. In fact, many of our Atlanta physical therapy patients pay less at Equalizer Health than they would through their in-network provider! Through our online patient portal, you will receive a “superbill” which contains all of the documentation needed to submit to your insurance company for reimbursement.
What types of conditions do you treat?
Spinal discomfort (lumbar, cervical, thoracic, etc.) might be the most common conditions we treat, but we also do orthopedic manual therapy for a variety of joint, knee, hip, shoulder, and ankle pain/injuries.
